You may be aware that general practice is contractually obliged to give patients access to book online appointments and the ability to order repeat medication online.
From April 2016 we are also contractually obliged to offer patients access to all of the coded data which is held on their medical record which includes medical problems, medication, allergies, test results and vaccination records.
If you wish to access your medical record securely on line you will need to call at the surgery to register to use this service. Please note, this process applies even if you are already registered to use our online access to appointments and repeat medicines. You will need to bring in one form of photographic ID such as driving licence or passport and proof of your permanent address (which must be within our practice area) such as a recent utility bill or bank statement (dated within the last 3 months). All applications must be made in person and you will also be given further information about the service.
Please allow 28 days from receipt of your application for your record to become available on line. If you are already registered for online services such as ordering your repeat medication and appointment booking, your coded medical record will appear automatically once your doctor has approved your application. If you are not registered for any online services we will contact you with a unique PIN code and instructions on how to activate the service.
